Background/Objectives:

To evaluate clinical significance of FDG PET-CT for detection of residual cancer cells after curative radiation therapy or chemoradiotherapy for patients with squamous cell carcinoma (SCC) of Head and Neck

Materials & Methods:

A retrospective analysis of patients with SCC of Head and neck with curative radiotherapy or chemoradiotherpy between June 2011 and Jan. 2019 was performed. Sixty patients were treated with Intensity-modulated radiotherapy (IMRT). The Metabolic responses were evaluated on the post-treatment FDG PET-CT at 12 weeks after curative radiotherapy completion.

Results:

Median follow up was 51.5 months (3-102). The overall survival (OS), disease free survival (DFS), local control rate (LCR), and Distant metastasis free survival (DMFS) at 5 years were 80.5%, 80.1%, 87.7% and 89.1%. Metabolic CR was found in 43 (71.7%) and partial metabolic response (PR) was noted in 17 (14.6%). Metabolic CR was significantly correlated with OS, DFS, LCR, and DMFS. On multivariate analysis, Metabolic CR remained significant for DFS and LCR.

Conclusion:

Metabolic CR on post-radiotherapy FDG PET-CT is highly predictive of increased DFS and LCR in patients with head and neck cancer.

서론

두경부암은 예전에는 담배나 술에 의하여 주로 발생한다고 알려져 있었으나, 최근에는 인유두종 바이러스(Human papillomavirus, HPV)가 원인인 경우가 점차 증가하고 있다. 1) 완치 목적의 치료방법에는 수술적인 절제술과 방사선치료 단독요법, 항암치료와 방사선치료 병합요법 등을 꼽을 수 있고, 대부분의 경우 하나의 치료방법 단독으로 사용하는 경우보다는 여러 치료방법을 병합하는 요법이 주로 사용되고 있다. 최근에는 방사선치료 기기와 방사선치료 기법의 발달로 인하여, 두경부기관의 기능 유지 및 미용적인 목적으로 인하여 방사선치료 단독 혹은 항암치료와 방사선치료 병합 요법을 먼저 사용하는 경우가 늘고 있어, 치료 후 환자의 예후를 예측하기 위하여 많은 방법들이 사용되어왔다.

최근에는 항암방사선치료를 끝난 후 12주 경 시행한 양전자방출촬영(8F-fluorodeoxyglucose positron emission tomography-computed tomography, FDG PET-CT)가 재발을 예측하는 민감도와 특이도가 각각 92%, 87%에 이른다는 보고가 있고, 2) 최대 표준화섭취계수값(maximum Standardized Uptake Values, mSUV)을 측정하였을 때 치료 전과 비교하여 완전관해(Metabolic complete response, Matabolic CR)에 이른 환자에서 림프절 곽청술이 필요하지 않을 가능성을 예측한 값이 97%에 달한다고 보고하고 있다. 3)

본 연구에서는 후향적으로, 방사선치료를 시행한 두경부암 환자에서 치료를 마친 후 12주 되었을 때 시행한 FDG PET-CT가 환자의 예후를 예측하는데 어떤 의미를 가지고 있는 살펴보고자 하였다.

대상 및 방법

2011년 6월부터 2019년 1월까지 두경부암으로 진단받고 항암방사선치료를 시행한 344명의 환자를 대상으로 하였다. 모든 환자에게 치료 전 진단을 위하여 영상 검사(컴퓨터단층촬영 혹은 자기 공명영상, 양전자방출촬영)을 시행하였고, 모든 환자에게서 조직학적 검사로 두경부암을 확진 하였다. 환자의 특성은 Table 1에 요약하였다.

Patients Characteristics

모든 환자는 세기조절 방사선치료를 시행 받았고, 이전의 영상학적 검사 및 조직학적 검사로 실제 암이 있는 부분(Gloss tumor volue, GTV)는 6750 cGy, GTV가 포함된 목의 림프절 레벨은 고위험 임상적 표적 용적(High risk Clinical target volume, High risk CTV)으로 정의하여 6000 cGy, 그 외 임상적으로 High risk CTV에서 위 아래 경계를 따라서 2-3cm 정도의 영역을 포함하고, 임상적 구조물의 관계에 따라 여유 공간을 포함한 저위험 임상적 표적 용적(Low risk Clinical target volume, Low risk CTV)에 5400 cGy의 방사선치료를 계획하였다.

항암치료는 4명(6.7%)를 제외한 56명(93.3%)의 환자에게서 시행되었고, 48명의 환자는 3주 간격으로 Cisplatin 100mg/m2을 정맥으로 투여하는 요법을 사용하였고, 5명의 환자에서는 Cetuximab, 3명의 환자는 1주 간격으로 40mg/m2의 Cisplatin이 사용되었다. 1명의 환자는 전신적인 상태가 ECOG (European Cooperative Oncology Group) 2점으로 항암치료를 생략하였고, 3명의 환자는 자의로 항암치료를 거부하여 시행하지 않았다.

모든 환자에서 방사선 치료가 시행되고, 1개월 후 신체검진을 시행하였고, 추가로 2개월 후 컴퓨터단층촬영을 시행하여, 치료에 대한 반응을 평가하였다. 치료 반응 평가는 modified RECIST criteria를 사용하였다. 양전자방출촬영은 모든 환자에게서 치료 전 및 치료를 모두 마친 후 3개월 때 각각 1회씩 시행되었고, 모든 환자는 양전자 단층 촬영 전 최소 6시간의 금식을 시행하였고, 촬영 전 혈중 포도당 수치를 측정하여 200 ml/dL 이하인 환자에게만 18F-FDG를 주사 후 촬영을 시행하였다. 얻어진 양전자 단층 촬영에서 최대 표준화섭취계수값(maximum Standardized Uptake Values, mSUV)을 측정하였다. 최대 표준화섭취계수값이 이전 양전자단층촬영과 비교하여, 병변(주병변과 전이로 보이는 경부림프절 포함)에서 측정된 값이 주변조직과 비슷하게 측정된 경우를 완전관해로 정의하였고, 30% 정도 낮은 값을 보이는 경우를 부분관해로 정의하였다. 4)

통계는 Statistical Package for Social Science (SPSS) 26.0을 사용하였고, 전체생존(Overall survival, OS)은 방사선 치료 시작일부터 사망까지의 시간으로, 무질병생존(Disease free survival, DFS)은 방사선 치료 시작일부터 재발 혹은 사망까지의 시간으로 정의하였다. 국소 무재발 생존률(local control, LC)은 방사선치료를 시행한 간종양에서 종양이 재발하거나 다시 진행하였을 경우, 원격 무전이 생존률(Distant metastasis free survival, DMFS)은 타 장기에 전이가 발견되었을 경우로 정의하였다. 위의 모든 값들은 Kaplan-Meier 생존분석을 통하여 산출하였고, 다변량분석에서는 Cox regression models가 사용되었다.

결과

전체 환자는 60명으로 남녀가 각각 53명, 7명이었으며, 평균나이는 64세(44-86)였다. 최소 추적관찰기간은 3개월이며 최대추적관찰기간은 102개월이며, 평균 추적관찰 기간은 48.1개월(중앙값: 51.5개월)이었다(Table 1). 전체생존률은 5년에 80.5%였고, 무질병생존률은 5년에 80.1%, 국소 무재발 생존률은 5년에 87.7%, 원격 무전이 생존률은 89.1%였다(Fig. 1). 방사선치료 후 시행한 컴퓨터단층촬영에서 완전관해가 43명(71.7%), 부분관해가 17명(28.3%)이었다. 치료 후 3개월에 시행한 양전자방출촬영에서는 완전관해가 43명(71.7%), 부분관해가 17명(28.3%)이었다. 양전자방출촬영에서 완전관해가 있었던 군에서 부분관해가 있었던 환자에 비해서 모든 치료 성적(전체생존률, 무질병생존률, 국소 무재발 생존률, 원격 무전이 생존률)에서 통계학적으로 유의한 차이를 보였다(Fig. 2). 반면 컴퓨터 단층촬영에서는 완전관해가 있었던 환자군과 부분관해를 보였던 환자군 사이에는 치료 성적에서 통계학적으로 유의한 차이를 보이지는 않았다(Fig. 3).

전체생존률에 대한 다변량분석을 시행하였을 때, ECOG 점수(p=0.004, 95% Exp (B)=20.346, CI 2.656-155.830)와 T 병기(p=0.010, Exp (B)=4.170, 95% CI 1.402-12.400)가 독립적 중요성을 보여주었고, 양전자 단층 촬영에 대한 반응(p=0.144, Exp (B) 0.374, 95% CI 0.100-1.398)는 통계학적으로 유의한 차이를 보이지 못했다. 하지만, 무질병생존률과 국소 무재발 생존률에서는 양전자 단층 촬영에서 완전관해를 보이는 것이 통계학적으로 독립적 중요성을 보였다(p=0.008, Exp (B)=0.137, 95% CI 0.032-0.592, p=0.020, Exp (B)=0.132, 95% CI 0.024-0.722)(Table 2).

고찰

두경부암 환자에서 경부림프절에 재발하거나 잔존암이 있는 경우에는 2년 생존률이 30%를 넘지 않는다고 보고되고 있다. 5,6) 따라서 재발암이나 잔존암을 조기에 발견하는 것은 환자의 생존률 높이는데 중요한 요소이며, 치료 후 환자의 치료 반응을 평가하고 예측하는 것은 추후 있을지도 모르는 잔존병변이나 재발을 조기에 발견하고, 2차적인 수술을 계획하는데 중요하다. 하지만 두경부암 환자에서 항암방사선치료를 시행 한 후 2차적인 수술을 하는 것은 방사선으로 인한 정상조직의 손상과 이러한 손상으로 인하여 수술 후 상처 회복에 걸리는 시간을 길게 하고, 정상적인 회복을 저해하여 환자가 장기적으로 부작용을 겪을 가능성이 높다. 7) 그러므로, 항암방사선치료를 마친 환자에게서 잔존병변을 정확하게 평가하는 것이 추후 치료 방침을 결정하고, 불필요한 2차 치료를 줄이며, 환자의 생존률에도 영향을 줄 수 있는 중요한 요소이다.

하지만, 항암방사선치료를 마친 환자에서 기존의 컴퓨터 단층 촬영이나 자기공명영상은 방사선치료에 따르는 염증성 반응과 잔존암을 구분하는데 어려움을 가지고 있다. 반면 PET-CT는 암세포가 포도당 펌프(Glucose transporter)와 당 분해 능력을 통해 포도당의 이용을 효과적으로 증대시키는 특징을 가지고 있고, 8) 이러한 특성 때문에 높은 최대 표준화섭취계수값이 종양의 악성도와 연관이 있을 수 있고, 9) Minn 등은 37명의 두경부 편평 세포암 환자에서 높은 최대 표준화섭취계수값이 불량한 예후와 관련이 있다고 보고 10) 한 것에 부합한다. 또한 방사선치료 계획을 세우는데 PET-CT를 사용하는 것이 종양의 국소제어율을 올리고 생존률을 향상하는데도 도움이 된다고 보고하고 있다. 11) 이 부분에서 PET-CT는 치료 반응 평가에 좀 더 많은 정보를 주어, 정확도를 향상 시킬 수 있다. 12) 방사선치료를 마친 후 4주 후 시행한 PET-CT의 사용이 치료의 반응 평가에 도움이 된다고 보고하고 있고, 치료 중 시행한 PET-CT에서 원발 종양의 최대 표준화섭취계수값이 치료 전과 비교하여 줄어드는 차이가 환자의 종양 국소제어율과 생존률의 향상을 예측할 수 있다는 연구가 보고되었다. 12,13) 하지만 치료를 끝낸 후 1주일 지나서 시행한 PET-CT가 12주 후 시행한 PET-CT와 비교하였을 때 예측도가 떨어지고, 14) 반면 2주 후 시행한 PET-CT 결과값이 불필요한 목의 림프절 곽청술을 줄여준다고 말하고 있다. 이러한 결과는 계획하에 림프절 곽청술을 시행한 연구결과와 상호 검증하여 보고하고 있다. 15,16) 이러한 연구결과를 종합해볼 때, PET-CT는 방사선치료를 마친 암환자에게서 치료 결과를 예측하는데 높은 민감도와 특이도를 가지고 있고, 두경부암 환자에게서도 치료 후 경과 관찰에 중요하다고 예측할 수 있다.

본 연구에서도 두경부암 환자에서 항암방사선치료를 마친 후 3개월 지난 후 얻은 양전자 단층 촬영에서 최대 표준화섭취계수값이 완전관해를 보였을 때, 환자의 예후가 좋다는 결론을 얻을 수 있었다. 또한 다변량 분석을 통해서, 양전자 단층 촬영에서 완전관해를 보였을 때, 국소제어율과 무질병생존률이 통계학적으로 유의한 차이를 보였다. 즉, 기존에 컴퓨터 단층 촬영이나 자기공명영상 등으로 잔존암과 염증반응을 구분하기 어려웠다면, 양전자방출촬영을 시행하여 환자의 예후를 예측하는데 도움이 될 수 있을 것이라고 판단된다.

결론

이 연구는 몇 가지 한계점을 가지고 있다. 먼저 후향적 연구이기 때문에, 모든 환자가 같은 치료를 받은 것이 아니고, 환자의 특성이 동일하지 않을 가능성이 높을 것으로 보인다. 또한 치료 성적에 큰 영향을 주는 인유두종 바이러스 감염에 대한 검사결과가 많은 환자에게서 누락이 되어있어서 이 지표로 인한 선택적 편견(Selection bias) 가능성을 배제할 수 없다.

하지만 이러한 한계점에도 본 연구는 두경부암 환자에게서 항암방사선치료를 마치고, 12주 후 시행한 양성자 단층 촬영에서 얻은 최대 표준화섭취계수값이 완전관해를 보였을 때 종양의 국소제어율 및 무질병생존률이 좋다는 결론을 얻었다. 다만, 본 연구는 앞에서 언급한 여러 제한점이 있으므로 이러한 점을 검증하기 위해서는 향후 추가적인 연구가 이어져야 할 것으로 생각된다.
