Trans Abstract

Ganglioneuroma (GN) is benign neurogenic tumor arising from ganglia of the sympathetic nervous system. They are mostly found at posterior mediastinum, retroperitoneum, and adrenal gland, whereas only 1-5% occurred in the cervical region. GN usually present as a single, painless and slow-growing mass, but multiple cervical occurrences are extremely rare. An 80-year-old woman came to our clinic complained of posterior neck mass for three years. We performed surgical excision, and it was finally diagnosed as GN. We report the unique and rare disease entity with a brief literature review.

서론

신경인성 종양에는 그 기원에 따라서 신경초종, 신경섬유종, 신경절신경종, 후각신경아세포종 및 부신경절종 등으로 분류된다[1]. 이 중 신경절신경종 (ganglioneuroma)은 원시 신경능세포(primordial neural crest cells)에서 유래하며, 교감신경절을 따라서 국소화되어 발생된다[2]. 주로 후종격동, 후복강 및 부신 등에 호발하고[3], 두경부에서는 경동맥 공간 및 상경부 등에서 잘 발생하며, 그 빈도는 1~5% 정도로 드물다[4]. 주로 서서히 성장하는 무통성 단일 종물로 나타나고[5], 10대에서 40대 사이에 흔하다[6].

저자들은 후경부 종물로 내원한 80세 여자 환자에서 다발성 신경절신경종으로 최종 진단된 드물고, 특이한 임상양상의 증례를 경험하여 문헌고찰과 함께 보고한다.

증례

80세 여자 환자가 내원 3년 전부터 인지된 우측 후경부 종물을 주소로 내원하였다. 고혈압과 B형 간염 보균자이며, 흡연 및 음주력은 없었다. 종물은 4개월 전부터 크기가 점차 증가하였고, 저명한 통증 및 압통 등은 없었다. 신체 검사에서 우측 승모근의 일부가 돌출되어 있고, 그 전하방에 약 3×1cm 크기의 비교적 딱딱한 원형의 종물이 촉진되었다(Fig. 1). 경부에 다른 비정상적 종물은 촉지 되지 않았고, 전신증상도 없었다. 인후두 내시경에서 특이 소견 없었으며, 신경학적 검사도 정상이었다. 경부초음파 검사에서 우측 후경부에 3.6×1.2cm 크기의 내부에 혈류의 흐름이 있는 저에코 음영의 소엽상(lobulated) 종물이 관찰되었고(Fig. 2A),경부 전산화 단층 촬영에서는 우측 후경부에 4.0×1.3cm 크기의 주변과 경계가 명확하고 비균일하게 조영 증강되는 종물이 관찰되었으며, 주변에 다른 작은 종물도 존재하였다(Fig. 2B, C , D). 초음파 유도 하 미세세침검사에서 의미가 불분명한 소수의 비정형 세포 소견으로 보고되었고, 중심생검에서는 다량의 림프구 침윤을 동반하는 저분화 방추세포 종양의 소견이었다.

이상의 결과를 종합하여 신경인성 종양, 연조직 종양, 림프종 및 혈관종 등의 가능성을 염두에 두고 확진을 위하여 절제생검을 시행하였다. 종물이 촉지 되는 부위의 직상방에 피부절개를 하고 박리를 하였다. 종물은 교감신경의 분지로 추정되는 신경과 매우 심한 유착이 있었고, 출혈이 있는 양상으로 종물과 신경의 완전한 박리는 불가능하였다(Fig. 3). 또한 주 종물의 하방으로 작은 부종물도 관찰되었다. 동결절편 검사에서는 양성의 가능성이 높다고 보고되었다. 종물의 위치와 신경자극 시 승모근의 반응 등을 고려해 보았을 때, 부신경의 가능성은 적다고 판단하여, 신경을 포함하여 주변조직과 함께 종물을 제거 하였다. 병리육안 소견에서 3.5×1.4×1.2 cm 크기의 소엽상 종물 이었고(Fig. 4A), 현미경 소견에서는 국소 림프절 침윤이 동반된 성숙된 신경절 세포들이 관찰되고, 주변으로 길게 연장된 슈반세포가 다수 관찰되었고(Fig. 4B and C), 면역 조직화학 염색에서는 S-100단백질에 양성 소견을 보였다(Fig. 4D).이상의 병리 소견들로 신경절신경종으로 최종 진단되었다. 술 후 4일 째 경미한 경부 감각저하 이외에 신경학적 이상 소견 없이 퇴원하였고, 주기적 초음파 추적 관찰 중으로 술 후 1년 째 재발 없이 추적 관찰 중이다.

고찰

신경능세포(neural crest cell)로 부터 발생하는 말초 신경세포 종양은 분화 정도와 슈반(Schwannian)기질의 발달 정도에 따라서 신경모세포종, 신경절아세포종 및 신경절신경종 등으로 분류된다[7]. 신경절신경종은 비교적 드문 양성 신경종양으로 주로 흉복부에서 호발하고[4,6], 두경부에서는 약5% 정도로 비교적 드물다[4]. 27례의 신경절신경종의 보고에서는 경동맥 공간에서의 발생이 9례, 상경부공간이 5례로 가장 흔했고, 대부분 일측성이고, 양측성은 1례였다[4,8]. 두경부에서는 주로 교감신경계를 따라서 발생하지만, 후두, 인두, 미주신경 등의 결절 신경절에서도 발생할 수 있다[2]. 이런 경우에는 측인두, 후인두 공간, 부인두 공간 및 측두골 등에서 종물로 나타날 수 있다[7]. 60%가 20대 이하의 젊은 연령에서 나타나며, 여성에서 조금 더 흔하다[2].

대부분 초기에는 서서히 자라나는 종물 양상으로 우연히 발견되지만, 크기가 증가하면 연하장애, 이물감, 호흡곤란, 비충만감, 음성변화, 청력저하 및 호너증후군 등을 호소할 수 있다[2,5]. 신경절신경종의 일반적인 영상소견은 초음파에서는 균일한 저에코음영을 보이고, 경부 전선화 단층 촬영에서는 균일하고 경계가 명확한 종물로 나타나고, 자기공명영상에서는 T1 강조 영상에서 저신호, T2에서 고신호 강도를 보인다[5,7]. 신경절신경종에 특이 영상소견이 없으므로 침샘종양, 다른 신경인성 종양, 섬유육종, 횡문근육종, 악성림프종 등과의 감별은 어렵고[5], 병리소견으로 만 확진할 수 있다[4]. 병리학적으로 성숙한 신경절세포, 슈반세포, 방추세포, 점액성 기질과 신경절세포의 이형성증 소견 등이 관찰되고[8], 면역조직염색에서 S-100 단백질에 양성소견을 보이면 진단이 가능하다[4]. 임상양상으로 신경절신경종과 상완신경총 기원 종물과의 감별은 어렵지만, 전자는 비교적 경부의 표층에 잘 위치하고, 후자는 보다 심부에 흔하게 존재하며, 또한 후자는 크기의 증가에 따라서 상지의 운동기능 저하 같은 증상이 나타날 가능성이 전자 보다는 비교적 높다는 보고가 있다[6].

대부분의 신경절신경종은 외과적 절제가 치료 원칙이며, 종물의 크기와 위치에 따라 다양한 접근법을 사용할 수 있다[5]. 수술 시 종물이 신경과 유착이 심한 경우에 본 증례처럼 신경의 보존이 어려운 경우가 많지만, 재발을 낮추기 위해서는 완전절제가 추천된다[4]. 대부분 단일 종물 이지만, 본 증례 처럼 드물게 다발성으로 존재하기도 한다[5]. 예후는 대체로 양호하며, 신경절신경종 26명의 보고에서는 완전 절제를 시행한 23례는 재발 소견이 없었고, 부분 절제를 시행한 3례 중 1례에서 재발하였다[5].

저자들은 본 증례를 통해서 후경부에 서서히 성장하는 난원형 또는 구형 종물의 경우에 드물지만 신경절신경종도 염두에 두고 진단을 시행해야 하며, 수술 시 종물과 신경의 심한 유착 가능성을 염두에 두어야 한다는 교훈을 얻었다.
